What are the responsibilities and job description for the LPN/RN position at Windy Acres?
Windy Acres is looking to hire upto TWO- LPN/RNs for our facility.
The NURSE will have flexible scheduling that YOU CHOOSE during the week (M-F). Every other Saturday/Sunday 6a-2p is required on the floor completing direct care with the residents. During the week, the hours are ‘office hours’ that you choose which days you want to work (1-3 days during the week) that you will choose for your ‘block’. In addition, you choose the hours (preferred at least 5 hours/day, somewhere between the hours of 6a-5p on the days you choose).
A management position could be available after 6 months or complete understanding on operation and procedures of the entire facility and licensing laws if nurse is interested.
The facility houses up to 15 residents age 55 an older. The facility has 2 NAs each shift that complete assistance with ADLs and IADLs under the direction of an RN/LPN/LALD. Staff assist resident with bathing, grooming, ambulation, toileting, eating, repositioning, etc.
Responsibilities
- Provide health care services to residents
- Provide training to unlicensed personnel to the job with state required training and further ongoing assistance and education
- Medication Management of facility for residents: Administer prescribed medications, Set up prescribed medication for trained staff to administer, maintain stock and supply, etc. Communicate with providers regarding medication, clarification, etc
- Monitor reported patients information (vital signs, skin condition, wounds, refusals, etc) and follow up as needed
- Maintain patients records and document efficiently, constructively and report to supervisor as required
- Provide supervisions per task list
- Provide day to day communications with supervisor, NA, residents, family, MD, CM, etc
- On-call: answer NA questions related to the resident cares, condition, or questions. Work open shifts/partial shifts during staffing shortages. On-call hours are scheduled every other weekend Thursday 6a-Monday 6a in summer and Friday 6a-Monday 6a in winter as a minimum requirement.
- Other tasks/duties as requested.
Employee Must:
- Able to work every other weekend as condition of employment
- This position is on-call on the same weekend of 6a-2p ‘floor’ schedule
- Ability to pass a MN DHS background check
- Have reliable transportation to and from work
- Proficiency in English
- Communication skills and ability to follow instructions
- Licensed as an LPN in Minnesota
- BLS/CPR certified
Helpful, but not mandatory:
- Have interest or experience with Geriatric populations

Base pay: LPN:$24-29/RN$29-34. This means you won't make less than the base pay, you will never be sent home or hours cut due to a 'low census'!
Census pay: you get paid more money, the more residents the facility has!!
