Job Overview
We are seeking a dedicated and compassionate Case Manager to join our team. Duties
- Conduct comprehensive assessments to determine client needs and eligibility for services.
- Develop individualized case plans that outline goals and strategies for achieving them.
- Provide crisis intervention and management support to clients facing immediate challenges.
- Facilitate access to resources such as addiction counseling, behavioral health services, and workforce development programs.
- Monitor client progress and adjust case plans as necessary to ensure effective support.
- Collaborate with other service providers, including childcare services and child welfare agencies, to coordinate care.
- Utilize motivational interviewing techniques to empower clients in their recovery journey.
- Maintain accurate documentation of client interactions and case progress in compliance with organizational policies.
- Advocate for clients’ rights and needs within the community and service systems.
Qualifications
- Bachelor’s degree in social work, psychology, or a related field is preferred; relevant experience may be considered in lieu of degree.
- Experience in addiction counseling, crisis management, or behavioral health is highly desirable.
- Strong mentoring skills with the ability to motivate clients toward positive change.
- Excellent communication skills, both verbal and written, with an emphasis on active listening.
- Ability to work independently as well as part of a multidisciplinary team.
- Strong organizational skills with attention to detail in documentation and reporting.
