Position Overview:
The Cardiac Rehab Specialist provides therapy programs for patients who qualify for Cardiac or Pulmonary Rehab services. This caregiver provides direct patient care, including assessment, development of the care plan in response to provider prescription, treatment, documentation, and patient education to clients of all ages. This individual directs patient participation and progression per clinical best practice guidelines to restore, reinforce, and enhance patient performance in preventing disease and its associated risk factors. They facilitate the learning of skills and functions essential for adaptation to disease. The Cardiac Rehab Specialist is accountable for productivity regarding shared patient volumes and personal and professional growth. They should possess the skills to adapt to a dynamic team environment and be open to developing, promoting, and performing new programs to facilitate rehabilitation. Programs may include new covered diagnoses, new equipment available for clients, or new education methods. This individual is also responsible for student supervision.
Essential Duties & Responsibilities:
- Performs direct patient care and evidence-based practice safely, ethically, and legally.
- Develop and revise a plan of care based on the initial assessment, patient history, diagnosis, provider orders, protocol, risk stratification, expected outcomes, and patient needs and goals.
- Evaluate patient response to exercise, measure change and development, share findings, and make appropriate recommendations and referrals.
- Directs patient participation, progression, and discharge based on the plan of care and “incident-to” recommendations from the referring provider(s) in accordance with clinical best practice guidelines to restore, reinforce, and enhance patient performance in the prevention of disease and its associated risk factors.
- Document all skilled and medically necessary care thoroughly, timely, and accurately in accordance with departmental policy. Documentation, charges, and care must comply with regulatory requirements and departmental policy.
- Routinely communicates patient response to exercise to referring provider and alerts of any negative or unexpected response to exercise. May assist with acquiring physician orders and review of monthly statistics. Maintains patient and organizational confidentiality at all times.
- Actively participates in activities necessary to maintain the department’s national certification.
- Schedules, instructs, and connects patients for Holter/event monitoring with computerized reports to physicians as required.
- Prepares and performs electro-cardiology studies in cooperation with providers and Winona Health imaging staff, processing documentation for ordering and attending providers as required.
- Performs duties in accordance with departmental productivity standards. Independent in using time wisely.
- Promotes service excellence with patient scheduling and accommodation with appointments.
- Develop programs, complete projects, and assist the department in utilizing appropriate resources.
- Serves as a mentor to students.
- Serves on departmental and organizational committees as appropriate. Delivers a high level of customer service to all internal and external customers.
- Fosters and promotes the cardiopulmonary profession; attends professional continuing education courses and in-services required for professional certification.
- Participates in focus boards, metric tracking, and performance improvement processes for quality improvement.
- Other duties and responsibilities as assigned based on the facility's needs and requirements.

Skills and Experience:
Required:
- Bachelor's degree in Exercise Science, Cardiac Rehab, or a related field of study
- BLS and ACLS certification
- Internship in cardiopulmonary rehabilitation
- American College of Sports Medicine (ACSM) clinical exercise physiologist certification within 18 months of hire date
- Basic computer skills including Word, Outlook and Excel
- Good time management skills, good communication skills, the ability to adapt to changing situations, a positive attitude, excellent customer service skills, the ability to problem solve and suggest solutions, and the ability to function successfully in a team environment
Preferred:
- Two years of clinical experience in cardiopulmonary rehabilitation and experience in stress testing preferred
- Patient information database (Cerner, EMR, American Data)
