Turning Point is hiring for a Social Services Case Management position.
Employee will work as part of the Interdisciplinary team to develop an appropriate treatment plan to meet the needs of the patient. This position will be a part of the discharge planning process. They will provide group and individual therapy based on needs of each patient. They will work under the supervision of a Licensed Social Worker.
Qualifications for this position include willingness to learn, a desire to work with elderly patients and be a part of a team who is dedicated to providing the best care possible for our patients. A valid MS LPN license is preferred but will look at experience in the healthcare field.
This is a full-time position with benefits.
