What are the responsibilities and job description for the MEDICAL RECORDS Coordinator position at Wiregrass Ranch ASC?
Job Summary: Manages all medical records functions (as appointed by the Wiregrass Ranch Board of Directors) relative to the patients ASC visit.
Essential Functions of the Position:
Monitors completion of physicians operative reports
o Ensures all reports are created by physician, are complete and electronically approved and signed by physician prior to adding to ASC medical record system.
o Contacts physician or appropriate office staff to obtain missing reports or for corrections as needed.
o Notifies supervisor and Administrator of delinquent records.
Monitors pathology reports
o Ensures all pathology reports are received and logged in specimen log book. Reconciles reports received with lab requisition forms and log book.
o Confirms physicians electronic signatures on any required pathology reports.
o Contacts lab if reports are not received timely.
o Scans finalized pathology report into ASC medical records system.
Monitors completeness of entire medical record
o Regularly reviews all patient records to ensure they are listed as 100% complete. Electronically closes completed charts.
o Notifies appropriate Manager/Supervisor of incomplete workflows as shown in the electronic chart that are preventing completion/closure.
Properly catalogues and scans hardcopy chart documents under the correct patient and date of service.
Processes medical record requests
o Obtains & reviews records release to ensure appropriate authorization has been obtained.
o Releases records appropriately and timely.
o Documents release in electronic medical records system.
Compiles required # of charts per provider for purpose of peer review. Selects patients randomly and prints necessary documents from the electronic chart and gives to appropriate Nurse Manager for distribution to peer reviewer. Monitors return and completion. Notifies Administrator and/or Risk Manager with issues regarding return rate.
Follows all ASC policies pertaining to Medical Records. Maintains familiarity with these policies.
Additional Responsibilities:
Performs other incidental and related duties as required and assigned, to include other Front Office position duties (i.e. Check In, scheduling, chart prep, answering phones, etc.) as needed.
Maintains high level adherence to HIPAA regulations.
Maintains all required regulatory training and participates in mandatory drills and staff meetings.
Follows all department policies.
Physical and Mental Demands:
Ability to stand, sit, and move for extended periods.
Occasional bending, occasional 50-pound lifting, and reaching for supplies, or equipment.
Normal concentration and normal complexity of decision making
High level verbal and written communication skills
Above average ability to manage multiple tasks simultaneously
Occupational Exposure:
Low risk exposure to bloodborne pathogens and chemical hazards.
Job Qualifications:
High school graduate or GED equivalent
Strong customer service orientation
Basic computer proficiency
Excellent telephone etiquette
Current BLS, or completed within 90 days of hire
