Brief Description of Programs/ Project:

The Hub & Spoke Health Home program aims to connect individuals with services to help support their health and recovery. Once enrolled, an individual is paired with a support team that includes a Recovery Navigator, who understands substance use and recovery on a personal level. Recovery Navigators then act as a bridge and connect individuals to the appropriate behavioral and healthcare services to meet that person's unique needs. The Hub & Spoke Health Home team seeks out ways to strengthen each person's community, family, and social supports to help achieve the identified recovery goals.
