Position Overview: The Long-Term Care Registered Nurse (RN) provides professional nursing care to residents in a long-term care facility. This role focuses on promoting the physical, emotional, and mental well-being of elderly, disabled, or chronically ill patients who require ongoing care. RNs in long-term care are responsible for assessment, planning, implementation, and evaluation of patient care, ensuring that residents receive the highest level of care and comfort.

Key Responsibilities:

  • Resident Care:
    • Conduct comprehensive assessments of residents’ health, including physical, emotional, and social needs.
    • Develop, implement, and evaluate individualized care plans in collaboration with residents, families, and interdisciplinary team members.
    • Administer medications, treatments, and procedures as ordered by physicians.
    • Monitor and record vital signs, symptoms, and conditions, noting any changes.
    • Provide wound care, manage catheter care, and assist with mobility and personal hygiene as needed.
    • Coordinate with the healthcare team to optimize care plans, adjust medications, and manage care delivery.
  • Patient Education:
    • Educate residents and their families on health conditions, treatment plans, and preventive care measures.
    • Provide instruction on activities of daily living, including diet, exercise, and health maintenance.
    • Counsel families and residents on end-of-life care options and advance directives when necessary.
  • Documentation:
    • Maintain accurate, up-to-date records of all nursing care provided in residents’ medical charts.
    • Document changes in conditions, response to treatments, and any interventions.
    • Ensure compliance with facility policies, state regulations, and industry standards regarding documentation.
  • Collaboration & Communication:
    • Work collaboratively with other nursing staff, physicians, therapists, and social workers to provide holistic care.
    • Act as a liaison between families, physicians, and other healthcare providers to ensure coordinated care.
    • Participate in care conferences, case discussions, and training sessions.
  • Regulatory Compliance:
    • Ensure that all nursing practices comply with state and federal regulations, including health, safety, and infection control standards.
    • Maintain confidentiality of all resident health information in accordance with HIPAA guidelines.
    • Assist in preparing for state or federal inspections and surveys, ensuring the facility meets all regulatory requirements.
  • Supervision & Leadership:
    • Supervise and mentor licensed practical nurses (LPNs) and nursing assistants (CNAs).
    • Provide guidance on the proper care techniques and ensure high standards of nursing practice within the facility.
    • Oversee and manage staffing needs, shift assignments, and break schedules in collaboration with the charge nurse or director.


Qualifications:

  • Education:
    • Current Registered Nurse (RN) license in [state/country].
    • Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN) required.
    • Certification in Long-Term Care Nursing (such as the RN-BC or Geriatric Nursing certification) is a plus.
  • Experience:
    • Minimum of 1 year of experience in long-term care, geriatric nursing, or related clinical settings preferred.
    • Experience in managing complex patient needs, including chronic conditions, dementia care, or hospice care.
    • Previous supervisory or charge nurse experience is beneficial.
  • Skills:
    • Strong clinical assessment, critical thinking, and decision-making skills.
    • Excellent communication and interpersonal skills to interact effectively with residents, families, and the healthcare team.
    • Ability to manage multiple priorities in a fast-paced environment.
    • Proficiency in using electronic health records (EHR) and other clinical software.
  • Certifications:
    • Basic Life Support (BLS) certification required.
    • Advanced Cardiac Life Support (ACLS) certification may be required depending on facility policies.
    • Additional certifications in wound care, palliative care, or other specialties are beneficial.


Working Conditions:

  • The position involves working in a long-term care facility, which may include nursing homes, assisted living centers, or rehabilitation centers.
  • Nurses must be able to work flexible hours, including evenings, weekends, and holidays.
  • The role requires standing, walking, lifting, and assisting residents in daily activities.
  • Exposure to infectious diseases and bodily fluids is possible, and proper safety precautions are mandatory.
