Job Summary :
- Responsible for the assessment, planning, implementation, coordination, monitoring, and evaluation of services across the continuum of care to ensure quality patient outcomes and appropriate utilization of healthcare services.
- Responsible for supporting the healthcare team towards a smooth transition from one level of care to another in support of the patient / family.
- Performs other duties as assigned.
Minimum Requirements :
- Bachelor's Degree in Social Work or Certified Community Health Worker required.
- Knowledge of community resources, state and federal laws, and social service programs
- Flexibility, creativity, and initiative to work independently and cooperatively within a multidisciplinary team
