About the role
The Outcomes Coordinator plays a critical role in ensuring the smooth and efficient functioning of the outcomes program at Within Health. This position is responsible for overseeing all aspects of outcome measures, from admission through post discharge. This position is also responsible for overseeing and improving the patient experience, aiming to ensure positive outcomes and address patient needs via management of the satisfaction survey process.
What you'll do
- Completion and reporting of client outcome measures:
- Ensure that all clients are enrolled in the outcome measure database upon admission and that initial outcome measures are sent to the client upon admission to Within Health.
- Ensure that initial outcome measures are completed within 48 hours but no later than the 7th day of admission.
- Follow proper processes in the event that a client has not completed their outcome measures.
- Follow the same processes for monthly and discharge outcome measures.
- Lead the communication associated with obtaining outcome measures in an effective and reliable manner that unites the care team with a measurement based care orientation.
- Ensure that post-discharge measures are sent to clients at the appropriate time, conducting proper follow-up based on whether they do or do not complete them with the goal of improving overall outcome measure completion rates
- Completion and reporting of all family / loved one outcome measures:
- Ensure that all appropriate family / loved ones are enrolled in the outcome measure database once all ROIs are properly obtained and that corresponding outcome measures are sent to them at the designated timeframes
- Ensure that outcome measures are completed within 48 hours of being sent, but no later than 7 days after being sent
- Follow-up with families / loved ones who have not completed their outcome measures to ensure proper delivery occurred and that they understand the importance of the data being collected
- Ensure that all follow-up measures are sent out according to designated guidelines, including discharge measures and post-discharge measures
- Satisfaction Program Gather patient feedback through surveys and other methods, and analyze the data to identify areas for improvement, and implement changes to enhance the patient experience.
- Manage the patient satisfaction committee.
- In collaboration with Data Manager, create patient satisfaction dashboards that provide quantitative feedback for actioning.
- Solicit and maintain caregiver / loved ones satisfaction surveys and incorporate data into both the satisfaction committee and satisfaction dashboards
- Other duties as requested.
Qualifications
- Bachelor's degree in healthcare administration, business administration, or a related field. Relevant experience will be considered.
- 2 or more years of experience in healthcare operations management, preferably in a behavioral health setting.
- Experience with electronic health record (EHR) systems.
- Strong analytical and problem-solving skills.
- Excellent organizational and time-management skills.
- Strong communication and interpersonal skills.
- Proficiency in Google Workspace.
- Experience with data analysis and reporting.
- Knowledge of relevant regulations and accreditation standards (e.g., Joint Commission). Employees are required to read, review, prepare and analyze written data and figures, using a computer or similar, and should possess visual acuity.
Physical & Environmental Requirements
- This position answers and places telephone calls as well as video conferences and must be able to converse.
- Must be able to converse with colleagues via telephone and computer programs.
- Must be able to operate a computer and navigate applications within a smart-phone, iPhone, MacBook computer and/or tablet.
- Able to sit for the majority of shifts.
- Must have reliable internet connection.
- This is a work-from-home position. Work should be performed in a private, quiet space with minimal background noise.
The pay range for this role is:
55,000 - 70,000 USD per year(Miami, FL)
