What are the responsibilities and job description for the Birth Certificate Clerk - Health Information Management - Full-Time position at Womans Hospital?
The Birth Certificate Clerk obtains birth information from the patient by phone and by visiting patient rooms. Advises patient on state regulations regarding completion of birth certificates. Notarizes paternity affidavits in accordance with Louisiana law. Abstracts medical information from patient records and submits to the State. Submits birth information for every birth at Woman's Hospital and maintains a record thereof. Assists patients in resolving problems regarding the birth certificate by communicating with local and state registrars. Performs various duties assigned by the department management according to policies and procedures and the mission of Woman's Hospital.
Requirements:
- High school diploma or equivalent.
- Experience in a Health Information Management department or equivalent work in a related healthcare field preferred.
Responsibilities:
- Retrieves birth list and verifies against Labor & Delivery log and nursing unit census.
- Calls or visits patient to collect information for infant's birth certificate on next business day following infant's delivery.
- Assists patients in completing paternity affidavits and notarizes such documents in accordance with requirements imposed by the State of Louisiana.
- Advises patient of legal regulations concerning birth certificate completion.
- Abstracts clinical information into birth certificate and send to delivering physician to certify in accordance with LA state filing guidelines.
- Logs in any birth certificates returned by the local registrar within 24 hours of receipt.
- Composes letters to LA Office of Vital Statistics for completion or correction of original birth certificate.
- Any other duties as assigned by Woman's Hospital.
Schedule:
Full-Time; 40 hours per week
Monday - Friday (Rotating Weekends)
8:00 AM - 4:30 PM
Pay Range:
Hourly/Non-Exempt
$13.00 – $18.00
