Qualifications:
- MD/DO
- Current valid license or ability to obtain a medical license to practice medicine in the State of Florida
- Current DEA certificate or ability to obtain
- Board eligibility in OB/GYN (ABOG, AOBOG) is required; board certification is required within 5 years of achieving initial eligibility and ongoing employment is contingent upon this
- Current BLS required

- Surgeon must hold current Board Certification in Female Pelvic Medicine and Reconstructive Surgery (FPMRS) or have completed a ABOG certified Fellowship in FPMRS and be an active candidate for Gynecologic FPMRS certification by ABOG.
- Board certification is required within 5 years of achieving initial eligibility and ongoing employment is contingent upon obtaining board certification.
