Qualifications / Requirements:

• Board-certified OB/GYN with MD or DO degree.
• Unrestricted State Medical License.
• Unrestricted DEA License or ability to obtain.
• OB/GYN Board Certification required within 5 years of eligibility.
• BLS certification.
