We are seeking an energetic and dynamic Service and Support Administrator (Case Manager/Social Worker).

The Wood County Board of Developmental Disabilities (WCBDD) coordinates services for over 1,000 people with developmental disabilities in Wood County. Our mission is to support, empower, and inspire. The ideal candidate will be a highly self-motivated problem solver, who is a team player with time management and leadership skills.

The Service and Support Administrator will manage a caseload of individuals, be responsible for writing the individual service plan and monitor the services to ensure health and safety.

  • FLSA: Non-Exempt

  • Normal Working Hours:  M-F, 8:00am – 4:30pm 

 

 

Job Responsibilities:

  • Schedule and facilitate Individual Service Plan (ISP)/ Individual Family Service Plan (IFSP) meetings using a person-centered philosophy.
  • Develop and amend as needed the ISP/IFSP, including service authorizations for individuals based on assessed need and using the Interdisciplinary Team process.
  • Be responsible for implementing, documenting and monitoring ISP data including narrative, assessments, and complete other related paperwork/material necessary to meet state/federal regulations and Board policies/procedure.
  • Assess, coordinate and ensure implementation of services consistent with individual needs which are consented to in accordance with the Individual Service Plan.
  • Provide direction of all services and behavioral programs.
  • Ensure complete and accurate documentation of all programs and review of service delivery.
  • Ensure individual/guardian choice in the selection of providers, assisting as needed in accordance with HCBS guidelines (if Waiver individual) and/or Board Policies or Procedures.
  • Collaborate with local school districts and Early Intervention providers.
  • Act as liaison between other Board components, families, outside agencies, the community, and contracted service providers.
  • To provide information and assistance in securing services for eligible children and adults into educational, vocational, housing, and other community-based service.
  • Provide personal advocacy to eligible individuals through direct contact. Document all service and support activities through Targeted Case Management, based upon identified needs and services.
  • Provide information and referral services to all interested individuals. Coordinate services for eligible individuals through direct contact and to share information intra-agency as well as with other agency providers.
  • Review Unusual Incidents and track and monitor for trends and patterns.
  • Complete and/or assist individual/family/provider in completing all required documentation for initial Waiver enrollment packet, assuring that all required forms are consistent and complete before submitting to the Ohio Department of Developmental Disabilities (Waiver enrollees only).
  • Assist individual/family/provider in the coordination of services to individuals based on multifactored assessments (if Waiver recipient, in accordance with Waiver criteria).
  • Coordinate completion of initial and annual comprehensive evaluations/assessments in accordance with Waiver criteria (if Waiver recipient).
  • Maintain ongoing communication with, and provide, as appropriate, copies to individual/family/provider regarding status of enrollment packet, services being delivered, ISP revisions and redeterminations.
  • Maintain periodic contact and report, as needed, of ongoing communication with individuals/families/providers to assure that all services identified in the ISP are being received/provided.
  • Maintain knowledge of HCBS Waivers.
  • Provide technical assistance to individual/families/providers regarding billing, documentation, amended plans and redeterminations.
  • Assist in the development of success plan strategies and Positive Supports First, monitor the effectiveness of such programs, and attend Transdisciplinary Meetings.
  • Assist in completion of forms for restrictive measures to be reviewed by Human Rights Committee.
  • Share 24-hour on-call responsibilities with other Service and Support Administrators in crisis intervention and stabilization.
  • Service and Support Administrators must have knowledge of Major Unusual Incidents (MUI) and reporting criteria, along with completing necessary MUI paperwork.
