GENERAL FUNCTION
The CSP Case Manager functions as part of a coordinated case management team that provides treatment, rehabilitation, and support services for chronically mentally ill persons who are able to live and work in the community while receiving these services. The CSP Case Manager is responsible for developing and implementing detailed plans to meet the treatment and support needs of CSP Consumers. The majority of services are provided in the community, in non-office settings to maximize access and ensure clinical benefit.

RESPONSIBILITIES
A. ESSENTIAL FUNCTIONS
1. Provide recovery-based individualized case management services to adults with severe persistent mental illness acting as the single point of contact for assigned clients on caseload to ensure a continuous clinical relationship. Work collaboratively with consumers and the CSP treatment team in assessment and in developing integrated service plans. Use strategies including strengths assessment, establishing hope, and facilitating empowerment for consumers in individual and group approaches.
2. Within parameters established within DHS 63 and Medical Assistance guidelines, complete required documentation, to include but not limited to assessments, treatment plans, contact notes, functional screen eligibility, and training, and supervision logs.
3. Participate as a member of a multi-disciplinary team, participate in treatment planning meetings and ensure consumer involvement. Prepare CSP required documentation including In-depth Assessments and Comprehensive Treatment Plans.
4. Provide direct services in the community (non-office settings) including psycho-social rehabilitation activities, symptom management, daily living skills training, medication monitoring, community living skills training, recreational and educational groups for consumers utilizing community settings and resources.
5. Provide support and back-up coverage for the Crisis Intervention Case Manager including providing linkage and follow up activities to consumers enrolled in the certified Crisis Intervention program, completing mobile crisis and walk in assessment and response plans as needed.
6. Responsible to know and practice the Safety policies of the County. Perform all job tasks in a safe and prescribed manner.
7. Must be familiar with and exemplify the Wood County Core Values and serve with courtesy and respect in accordance with the Wood County Code of Conduct.

B. OTHER DUTIES
1. Provide generalized referral information to individuals and the community at large as needed.
2. Participate in continuing education via conferences and in-service training.
3. Provide consultation and education about mental health issues to service agencies and other community organizations in the community. Education topics to address stigma of individuals with mental illness and/or substance abuse issues, recovery-based services, integration of individuals with mental illness and substance abuse issues into the community.
4. Perform other functions as assigned by the CSP Program Manager.
5. Any other duties as may be assigned.

MINIMUM TRAINING AND EXPERIENCE TO PERFORM ESSENTIAL JOB FUNCTIONS:
A Bachelor’s degree in Social Work or a related behavioral science field (Sociology, Psychology, Vocational Rehabilitation, etc.), plus 1,000 hours of supervised post degree experience with chronically mentally ill persons is required. Or a Bachelor’s degree in a non-behavioral science field plus 2,000 hours of supervised post degree clinical experience with chronically mentally ill persons is required.

Knowledge and experience working with co-occurring (mental illness and alcohol and other substance abuse) disorders is preferred. Knowledge of recovery-based principles and services is desirable. Position requires the ability to work effectively with a variety of other persons and requires the ability to work and manage own time effectively. Good oral and written communication skills are vital. Candidates must possess basic everyday living skills and the ability to understand and follow directions, the ability to read and write (for reports), and the ability to interview and counsel. This position requires travel to and from off-campus sites, homes and various community settings. Possession of a valid driver’s license, as well as a licensed and insured automobile is required, or other means of reliable transportation immediately available to individual.
