CURRENT OPPORTUNITY TO JOIN OUR TEAM:
The Wood County Human Services Department has an immediate opening for a full-time (.97 FTE) Youth Mentor Case Manager Position. This position performs ongoing, supportive case management services to youth with justice system involvement ages 12-18. The Youth Mentor Case Manager is responsible for mentorship, technique-building and family systems case management for program participants, working collaboratively with participants to develop a strengths-based prevention plan that works to assist them in increasing resiliency, positive emotional vulnerability, and community building. The Youth Mentor is to provide a system of care that is youth-centered with the needs of the child and family dictating the types and mix of services provided, to assist in keeping the family together, and preventing an out-of-home placement. This position is youth-driven and focuses on developing youth strengths, promotes positive relationships and connections for youth participants, and provides the support and follow-up services needed to address the range of challenges participants confront.

To be effective, the Youth Mentor will work with children in their homes and communities, incorporating the entire family and community support partners in a wraparound process. This position will have a home office with which to work, but most of the Youth Mentor Case Manager’s day will be spent out of the office. The daily schedule will be variable based on youth and family availability and will require evening hours during the school year.

Essential Duties and Responsibilities

The following duties are normal for this position. These are not to be construed as exclusive or all-inclusive. Other duties may be required and assigned.
1. Provide holistic and comprehensive mentoring services to all participants. Including: intake, assessment, goal setting, progress monitoring, advocacy and referral.
2. Develop, maintain and follow goal plan for each child/youth served.
3. Set realistic expectations and goals based on each child’s strengths, talents, abilities and needs.
4. Provide enrichment resources and activities which include opportunities for cultural awareness, diversity, and community service.
5. Complete required entries into appropriate tracking database.
6. Maintain accurate and detailed accounts of all participation in the program using internal tracking systems.
7. Develop relationships with alcohol and other drug abuse prevention programs for continued relationship building and referral information.
8. Develop partnerships with community resources and agencies for community building and support of program participants. Work collaboratively with providers to provide successful outcomes for participants.
9. Teach life skills and healthy habits to contribute both to basic development and learning, and to contribute to building positive self-esteem.
10. Develop a talent/skill/area of interest with each child to help develop positive self-esteem and experience growing confidence.

Additional Tasks and Responsibilities

While the following tasks are necessary for the work of the unit, they are not an essential part of the purpose of this position and may also be performed by other unit members.
1. Promote substance use prevention through additional partnerships, participating in related coalitions and community groups as requested.
2. Promote skills training and provide reinforcement to promote substance use prevention through community events and trainings.
3. Apply trauma-informed care knowledge to mentorship practices.
4. Any other duties as may be assigned.

4. Excellent written and verbal communication skills, ability to establish rapport with program participants.
5. A strong sense of and respect for confidentiality involving both participant and fellow employees.
6. Ability to relate to children and to develop and sustain trusting, supportive relationships with them.
7. Ability to work positively when children express reactive behaviors and actions.
8. Proven background experience in working with children who may express heightened trauma response.
9. Ability to navigate between family experiences and child behavior with a strength-based approach.
10. Strong vision of hope and positive expectations when engaging all parties involved.
11. Schedule is non-traditional, but flexible and based around clients’ availability. Since case managers are heavily involved with each family, long hours may be required.
Minimum Training and Experience Required to Perform Essential Job Functions
A Bachelor’s Degree in Human Services, Social Work, Sociology or related field that provides the necessary skills to meet the expectations of the position. Knowledge of human behavior, motivation, crisis intervention, family and group dynamics, money management, and organizational skills are necessary. Interviewing and assessment skills are required. Participation in State or Agency training is required. Basic everyday living skills are needed, as is the ability to understand and follow oral and written directions. Reading, writing, adding, and subtracting skills are needed. Must have access to transportation to travel to various work sites. Basic office equipment used: personal computer, projector, printer/copier and telephone.
