Transitional Care Nurse The Transitional Care Nurse is responsible for supporting patients and caregivers during the critical 30-day post-discharge period, ensuring a smooth transition home from a higher level of care. This role includes telephonic outreach, care coordination, and follow-up support to improve patient outcomes and satisfaction. Transitional Care Nurse is a hybrid position.Essential Functions
1. Conduct routine telephonic outreach to patients and caregivers following discharge, assessing their ability to execute the discharge plan, ensuring follow-up appointments, and reviewing medication needs. Maintain accurate, timely, detailed reports and records.
2. Collaborate with providers, caregivers, and community resources to address social determinants of health, health education gaps, and barriers to care, offering self-management education and facilitating care coordination. Collaborate and advocate with other health care providers to provide best outcome for patients.
3. Documents accurately in the medical record patient care interventions, goals, and collaboration with other healthcare team members to monitor patient progress and satisfaction.
3. Prepare, prevent, protect and respond to support our culture of safety.
4. Works hours per week in accordance with offer letter or per attributes of position at transfer request. May be expected to work longer hours as necessary.
5. Other duties as assigned.
Requirements
Education: Diploma Nurse or ADN from an accredited school of nursing,
License: Ohio Registered Nurse
Experience: 2 years nursing experience
Certification:
Preferred
Education: BSN from an accredited school of nursing
Experience: Two years case management
Certification:
