Position Summary :

Woodmark Pharmacy is currently looking for a dedicated and customer-service oriented PER DIEM Clinical Pharmacist Consultant to join our dynamic team. Please note that this is an on-site position and not available for remote work.

Key Responsibilities :

  • Collaborate with the Director of Clinical Pharmacy Services & Education to create new resources while covering for team members on PTO.
  • Review medication for 50-60 beds daily, accounting for approximately 400-500 beds monthly during medication regimen evaluations.
  • Conduct audits in nursing stations, including medication room assessments, medication cart reviews, controlled substance documentation, and medication room storage inspections.
  • Evaluate changes in condition and new admission cases.
  • Engage in facility QAPI meetings to contribute to quality improvement initiatives.
  • Support DPH survey efforts.
  • Facilitate clinical initiatives, including antimicrobial, opioid, psychotropic, and anticoagulant stewardship programs.
  • Demonstrate strong customer service, effectively communicating with residents, staff, and visitors.
  • Exhibit excellent organizational skills, managing multiple responsibilities while ensuring accurate documentation.
  • Maintain a high level of attention to detail.
  • Proficiently use Microsoft Office and possess strong computer skills.

Qualifications :

  • BS RPh or PharmD degree.
  • 1-3 years of experience as a Long-Term Care (LTC) Consultant is essential.
  • Exceptional communication and customer service skills.
  • Massachusetts licensure is required.
  • This role involves regular interaction with residents, coworkers, and supervisors, necessitating proficiency in reading, writing, speaking, and understanding English at an intermediate or advanced level.
