Responsible to provide health and program specific information to clients of the Nursing Division under the supervision of nursing staff. Will deliver and perform designated aspects of various nursing programs to targeted clients. This may include working in multiple nursing programs, data collection and entry, supply monitoring and ordering, registering and screening clients, maintenance of client records, and entry of client data in program data-bases. This position is within Siouxland District Health Department’s Nursing Division, and reports to the Nursing Director and/or Nursing Coordinator.
Essential Functions and Responsibilities:

- Provide health information to clients on an individual and group basis.
- Schedule client visits and respective services related to specific program guidelines.
- Role model professional behavior to peers and clients.
- Enter client demographic and program specific data into designated data-bases or documents.
- Demonstrate professional phone and written communication skills.
- Maintain client records and documents according to program requirements.
- Assist in immunization clinics.
- Assist in the preparation of clinical records, division statistics and division reports as requested.
- Assist in division/agency planning and response to disaster and emergency situations or events in conjunction with agency emergency plan.
- Participate in agency evaluation and strategic planning activities.
- Maintain daily statistics to assist in cost report, division statistics and agency statistics.
- Attendance required.
- Perform other duties as assigned.

- Ability to communicate and interact well with clients, peers and supervising personnel.
- Ability to use a computer and related software programs including related technology equipment.

Minimum Education and Experience Required to Performing Essential Functions:

- Must be an Iowa resident or willing to relocate.
Special Requirements
Must be able to communicate well orally and in writing both in English. Mandatory Child and Adult Abuse Reporter Training required. CPR Certification required. Must have a valid driver’s license, reliable transportation and ability to obtain car insurance at agency-specified liability level. Must be an Iowa resident or willing to relocate. Candidate for hire must successfully pass a background check, a pre-employment physical examination and drug screening test prior to employment.
