We are a 143 bed Skilled Nursing Facility located in Port Jefferson Station, NY.We are seeking a Speech-Language Pathologist to provide services for disorders of speech, voice, swallowing and language. Plans appropriate Speech-Language Pathology services and provides treatment in accordance with accepted professional standards and participates in the general functioning of the rehabilitation therapy department.

Plans appropriate Speech-Language Pathology services and provides treatment in accordance with accepted professional standards and participates in the general functioning of the rehabilitation therapy department.
Responsibilities
- Documents resident progress toward Speech-Language Pathology services
- Complies with Federal and State regulations related to rehabilitation therapy.
- Demonstrates working knowledge of Medicare Part A PPS PDPM case management
- Complete all doumentation to comply with professional standard and regulatory requirements.
- Demonstrates working knowledge of skilled restorative therapy and maintenance programs.
- Screens residents for rehabilitation therapy needs.
- Completes evaluations upon physician order.
- Communicate and work together across departments.
- EducationMid Level (3-6 years)
- ExperienceBachelors Degree
