Temporary / Interim Social Services Director Opportunity
Salary : Starting at $25 / hour depending on experience
Woodland Manor Nursing and Rehab is seeking a Temporary Social Services Director for our nursing home in Elkhart, IN. This opportunity is ideal for candidates with skilled nursing / long-term care (LTC) experience who are looking for a short-term, impactful role in a supportive environment.
Key Responsibilities :
- Develop social assessments and care plans to address medically related social and emotional needs, setting realistic goals and actions.
- Actively participate in pre-admission and admission processes.
- Provide compassionate support to residents and family members during times of adjustment, crisis, or special needs.
- Document residents’ behavioral and psycho-social status effectively and accurately.
- Facilitate individual and group support for residents and their families.
- Create and maintain a volunteer program tailored to meet specific social service needs.
- Orient residents and families to the facility, ensuring a smooth transition and positive adjustment.
