We are seeking a dedicated RN or LPN whom will be responsible for tracking Infectious Diseases to join our healthcare team whom will be responsible for Antibiotic Stewardship, Infectious control and Wound Care. This candidate will have a passion for providing high-quality patient care.

Job Description:
*Asses all wounds to be reviewed upon admission and weekly and as newly found. Review for appropriate treatment that is ordered or if treatment is needed to be ordered; address with a provider and relay information to charge nurses so that continuity of wound care is maintained.
*Maintain working relationship with therapy/RNA/Dietary to address wound issues in a multidisciplinary fashion.
*Measure and stage wounds on a weekly basis ensuring documentation in a timely fashion and that any significant changes /new wound or non healing wounds are reported on a daily basis to the DON.
*Monthly review of all pressure relieving devices in the facility as well as efficacy and need. Report needs to therapy /provider/DON at the end of the month or sooner as indicated by need.
*Work in cooperation with ADON to monitor and review all wounds caused by falls or incidents that have caused wounds.
*Assist with difficult wounds and dressings with residents that are noncompliant or resistive to cares.
*Update family members and residents of any new orders or changes in wound care as orders are received from care provider.
*Address any additional wound cares or needs as deemed appropriate by DON.
