What are the responsibilities and job description for the Family Worker position at Woonsocket Head Start Child Development Association?
CORE RESPONSIBILITIES:
•Facilitate care coordination for all assigned families through development of family assessments and individual family plans.
•Take a strength-based approach to help families goals and assess progress toward these goals for assigned caseload, advocating for families to ensure they receive appropriate entitled services.
•Conduct a minimum of 3 home visits per program year with assigned families; make referrals to appropriate resources and follow-up to ensure referral is acted upon; respond to family/child crises as appropriate. Maintain at least monthly contact with all assigned families.
•Establish and maintain cooperative relationships with community services and agencies; advocate for families to ensure they receive entitled services.
•Lead and facilitate groups for parents and/or children.
•Maintain confidentiality and obtain appropriate releases when sharing information about children and families.
•Encourage parent participation in all aspects of the program: parent committee, educational activities, support groups, policy council, component committees, and as volunteers.
•Maintain confidential, detailed records of all referrals, follow-ups, home visits and other contacts, and personal information changes; record and compile various statistical data for reporting purposes.
•Participate in staff development and training: attend staff meetings and in-service/pre-service training sessions; attend a minimum of 2 outside job-related workshops; learn and model Head Start philosophy and objectives.
•Assist families with transition into/out of the Head Start program.
•Maintain two-way communication with both the child’s regular Head Start teacher and the extended day care staff to assure child’s experiences at WHSCDA are seamless.
•Use a team approach in working with classroom and department staff.
•Communicate about attendance, health and home life situations that might affect the child.
•Include teacher and extended day care staff’s perspective in case conferencing (intensive case conferencing) and referrals for mental health, special needs, etc.
OTHER RESPONSIBILITIES:
•Assist with recruitment and selection of program participants
•Mentor newly hired Family Service staff.
•Tracking data on computer, including generating monthly statistics
•Supervision of children is everyone’s responsibility
•Perform other related duties as assigned.
