Currently, we are looking for a Shared Living Case Manager who will support individuals served in our Shared Living Program by promoting independence while empowering and enhancing their quality of life. Supervision and support from the Director of Shared Living Services.
The ideal candidate for this position is a self-motivated, energetic, team-player who enjoys working with and collaborating with various professionals, diverse individuals served, and family members.
Hours:
The position is 40hrs, Monday to Friday 10 am to 6 pm (some flexibility), with one Saturday per month required. This position includes frequently visiting the Shared Living homes throughout our service area and providing consultation and support to individuals and providers.

Responsibilities Include:
- Participate in the screening process for potential providers, identifying matches and transitioning clients into the Shared Living placements
- Visit and monitor shared living placements minimally once per month and complete Home Visit reports. Complete bi-weekly phone check-ins.
- Ensure providers meet training requirements including CPR/First Aid, Human Rights, Safety etc.
- Monitor placements for quality and individual satisfaction.
- Identifies needed changes and emergent needs on an ongoing basis
- Assist in the development of quality services and engaging activities that meet the individual’s needs
- Ensure that all DDS documentation has been arranged and set up with the provider at the home to meet all ISP and clinical requirements.
- Provide on-call support and assume the role as primary on-call at scheduled times.
- Maintains consistent communication with funding sources and outside providers
- Assists individuals in community integration.
- Report any concerns or issues to the Shared Living Director in a timely manner.
- Directly supports up to fifteen individuals.
- Ensuring the health and safety of each individual.
- Carries out supervisory responsibilities in accordance with Work Inc.’s company policy and applicable laws.
- Address complaints and resolve problems

Qualifications

- Possession of a valid driver's license and acceptable driving record. Must have own transportation.
- Related college degree and/or a minimum of one plus years’ experience or High School Diploma with 3 to 5 years of experience in a related field.
- Experience and/or familiarity with Shared Living, AFC (Adult Foster Care), or other residential service models (preferred)
