Job Description
Care Manager LCSW for the Care Management Team in Torrance, CA. This is a full-time position that will work 8-hour Day shifts.
Care Management within the Department of Psychiatry includes a collaborative practice with patients, caregivers, nurses, social workers, physicians, payers, support staff, other practitioners and the community. Social workers facilitate communication and care coordination along a continuum through effective transitional care management. Recognizing the patient's right to self-determination, the significance of the social determinants of health and the complexities of care (barriers to coping, abuse, mental health, substance use, lack of resources, psycho-social issues, etc.).
Required Qualifications :
- Master's Degree - Social Work.
- Coursework / Training - All LCSWs must follow the guideline by Los Angeles County Department of Mental Health for LPS designation and keep their designation in current status.
- California Clinical Social Worker License upon hire.
- 2 years experience - Healthcare related setting with psychiatric mental health patient population in area of service, preferably in an inpatient setting.
- Computer experience.
- Designated as Behavioral Health Clinician to perform 5150 evaluations 18 months of hire.
- CPI certification (nonviolent crisis intervention training) within 1 year of hire.
