Job Summary:
The Wound Care Nurse Practitioner (NP)/Physician Assistant (PA) provides specialized wound care services to patients in home settings, nursing homes, and assisted living facilities. This role involves direct, onsite care for patients with chronic wounds, pressure injuries, surgical wounds, and other complex skin conditions. The NP/PA is responsible for performing comprehensive assessments, developing and managing individualized care plans, educating patients and caregivers, and working collaboratively with healthcare professionals across various settings to achieve optimal healing outcomes.
Qualifications:
- Valid PA & NP License
- Credentialed with Medicare
- Minimum 2 year of Wound Management Experience or Related Field
- Proficient in Wound Management Principles, techniques, and best practices
- Experience working with diverse populations
- Familiarity with medical record systems and documentation requirements
- Demonstrate proficiency in using mobile devices, applications, and camera functionalities.
Responsibilities:
- Deliver specialized wound care treatment and management in various settings
- Assess, document and take pictures of patients' wounds, capturing characteristics, measurements, and progress
- Develop and implement individualized wound care plans based on patient needs
- Administer appropriate wound care treatments, encompassing dressings, debridement, and topical medications
- Monitor and manage wound healing progress, adapting treatment plans as necessary
- Collaborate with physicians, surgeons, and other healthcare professionals to optimize patient outcomes
- Educate patients and families on proper wound care techniques and prevention strategies
- Maintain accurate medical records and ensure compliance with regulatory standards
- Order and interpret lab and diagnostic testing
- Follow up on patients to assess the effectiveness of weekly amnion membrane treatments
Physical Requirements:
- Requires physical effort most of the day including kneeling, squatting, twisting, climbing and walking.
- Able to lift and/or transfer up to 50lbs of equipment
Requirements:
- Must have a valid driver’s license and minimum state required liability auto insurance; local travel is required.
- Must have a reliable transportation.
Compensation:
- Payment structured per encounter: $100-$250 per encounter.
- Drive time compensation
- 1099 Contract Position.
- Partial Mileage Compensation.

Pay: $100.00 - $250.00 per hour
Expected hours: No less than 16 per week
