Position Overview:
We are seeking an experienced Advanced Wound Care Nurse Practitioner to join our mobile healthcare team. As a key member of our company, you will play a critical role in assessing, treating, and managing complex wound cases in various clinical settings outside of traditional hospital environments. This position offers a unique opportunity to provide specialized wound care services directly to patients where they reside, ensuring continuity of care and promoting healing outcomes.
Responsibilities:
- Conduct thorough assessments of patients with acute and chronic wounds, including wound evaluation, measurement, and documentation.
- Develop and implement individualized wound care treatment plans based on evidence-based practices and patient-specific needs.
- Perform wound debridement, dressing changes, and other advanced wound care procedures using sterile techniques.
- Educate patients, caregivers, and healthcare staff on wound management, prevention strategies, and self-care techniques.
- Collaborate closely with physicians, nurses, therapists, and other healthcare professionals to optimize patient outcomes and continuity of care.
- Utilize telehealth and mobile technology platforms effectively to support remote consultations and follow-up care.
- Maintain accurate and timely electronic health records (EHR) documentation in compliance with regulatory standards and company policies.
- Participate in interdisciplinary team meetings, case conferences, and continuing education activities to enhance clinical knowledge and skills.
Qualifications:
- Master's degree or higher in Nursing from an accredited Nurse Practitioner program.
- Current national certification as a Nurse Practitioner (NP) with specialization in wound, ostomy, and continence nursing (WOCN) preferred.
- Active state licensure as a Nurse Practitioner and current certification in Advanced Practice Registered Nurse (APRN).
- Minimum of 1 years of clinical experience in wound care management, preferably in home health, long-term care, or outpatient settings.
- Proficiency in performing wound assessments, debridement, and advanced wound care procedures.
- Strong clinical judgment and decision-making skills with the ability to work independently and as part of a collaborative team.
- Excellent communication, interpersonal, and patient education skills.
