As a Wound Care Nurse Practitioner, you will play a vital role in assessing, diagnosing, and treating patients with acute and chronic wounds. You will work independently in the field, performing sharp debridement and administering advanced treatments, while collaborating with our multidisciplinary team to ensure exceptional patient outcomes.
- Conduct comprehensive patient assessments and document medical histories in EMR.
- Perform wound care procedures, including sharp debridement and application of amniotic allografts.
- Order and interpret lab tests and diagnostic studies.
- Develop individualized treatment plans based on clinical findings.
- Monitor treatment effectiveness and adjust care plans accordingly.
- Collaborate with physicians, nurses, and home health teams to coordinate care.
- Maintain accurate and timely documentation.
- Travel between patient locations as needed.
- Board-Certified Nurse Practitioner or Physician Assistant licensed in California.
- Minimum 1–3 years of wound care experience preferred.
- Prior experience with debridement is required.
- Strong knowledge of wound healing, infection control, and clinical documentation standards.
- Excellent communication and interpersonal skills.

- Valid driver’s license and clean driving record
- Clinical expertise in wound management.

- Mobile field-based role with some in-office responsibilities.
