Woundology is seeking a dynamic and experienced Case Manager to join our team. The ideal candidate will possess a deep understanding of the healthcare industry, particularly in wound care management and Transitional Care Managment. This role is crucial for expanding our reach and establishing strong relationships with medical accounts as they will be Woundology's representative in our partner hospitals. A Case Manager of Woundology acts as an advocate for the patients and is the connection between the patient and the medical provider to ensure that services and patient needs are met by managing and coordinating the patient’s care plan. Their main responsibility will be working hand-in-hand with the hospital case managers to capture patient referrals prior to discharge and arrange initial visits by our mobile medical providers in their homes for a successful discharge home.
Key Responsibilities :
The Case Manager with Woundology provides care management and coordination of services including home health orders, physical therapy, imaging, and lab orders for patients who are home bound through designated outside agencies. To accomplish the coordination of these services, the roles and responsibilities include :
Monitor inbox in electronic medical record for new or incomplete orders
Utilizes the electronic medical record to process orders for labs, imaging, home health services, and DME orders, sends them to the appropriate outside agency for completion, and ensures outside agencies follow through with requests
Update Smartsheets data sheet with completed orders and any patient communication that is pertinent to continuity of care
Establish rapport with Hospital staff and case managers to provide services to the patient
Collaborates with medical providers to ensure clinical notes are consistent with orders requested for care
Communicates with patients, their family’s, and medical providers when needed to ensure care continuity and manage barriers in care.
Provides feedback to medical providers on quality documentation to support the patient’s medical conditions and order requests
Follow up with Hospitals and Home Health companies on mutual patients weekly and PRN
Complete site visits to Hospitals with case managers for new referrals
Assist in marketing meetings and networking events as needed
1 Assist with insurance and social issues
Assist patient care coordinators with Smartsheets data sheet patient information data entry, electronic medical record data entry, and communicating with Home Health companies, patients, and patient scheduling
1 Any other duties assigned as needed
Requirements :
- Proven experience in Case Management, preferably within the Hospital System, with a focus on wound care or related fields.
- Established relationships with medical accounts and healthcare providers.
- Exceptional communication and interpersonal skills, capable of building trust and rapport with clients.
- Reliable transportation is required for travel to Hospital locations.
- Ability to work independently and as part of a collaborative team.
