Job Summary :
We are seeking a skilled and compassionate Mobile Wound Care Nurse Practitioner (NP) or Physician Assistant (PA) to join our healthcare team. In this role, you will provide high-quality, patient-centered wound care services directly in patients' homes, long-term care facilities, or other outpatient settings. You will assess, diagnose, and treat a variety of wound types, including chronic, complex, and acute wounds, with the goal of promoting healing, preventing complications, and improving patient outcomes.
As a mobile provider, you will work independently and as part of a multidisciplinary team to deliver care that is both effective and empathetic, focusing on holistic patient care while fostering collaboration with family members, caregivers, and other healthcare professionals.
Key Responsibilities :
Conduct Wound Assessments :
Perform thorough assessments of chronic, acute, and complex wounds, including pressure ulcers, diabetic foot ulcers, venous leg ulcers, surgical wounds, and other types of skin breakdown.
- Utilize appropriate wound classification and documentation systems, such as the PUSH (Pressure Ulcer Scale for Healing) tool or others, to track healing progress and outcomes.
Develop Individualized Treatment Plans :
Create personalized wound care plans based on patient assessment, incorporating evidence-based practices and current wound care guidelines.
Patient Education :
Provide education to patients, caregivers, and families on proper wound care techniques, infection prevention, and self-care strategies.
Pain Management :
Assess pain levels and implement pain management strategies to ensure patient comfort during wound care procedures.
Documentation :
Maintain accurate and up-to-date documentation of patient assessments, treatment plans, wound progress, and patient interactions in accordance with organizational and regulatory standards.
Follow-Up Care :
Schedule and conduct follow-up visits to monitor wound healing, reassess treatment plans, and address any complications or concerns.
Clinical Leadership :
Serve as a resource for patients, caregivers, and other healthcare providers regarding wound care best practices and advanced wound care techniques.
Qualifications : Education :
Master’s degree in Nursing (for Nurse Practitioners) or a graduate degree in Physician Assistant studies from an accredited program.
Licensure :
Current, valid licensure as a Nurse Practitioner or Physician Assistant in the state of [Insert State].
Experience :
At least 2 years of experience in wound care, ideally including experience in a mobile or home healthcare setting.
Skills :
Excellent wound assessment and clinical judgment.
