Job Summary :
We are seeking a dedicated and compassionate Mobile Wound Care Nurse Practitioner (NP) to join our team. The ideal candidate will provide specialized wound care services in the homes of patients, in nursing homes, or other healthcare facilities. This position requires a strong clinical background in wound care, advanced nursing skills, and the ability to work autonomously while providing high-quality patient care in a variety of settings.
Key Responsibilities :
Wound Assessment & Management :
Perform comprehensive assessments of patients’ wounds, including size, type, and stage of healing.
- Develop and implement individualized care plans for managing chronic, acute, and surgical wounds.
- Perform advanced wound treatments such as debridement, dressing changes, and pressure ulcer prevention.
- Monitor wound healing progress and adjust treatment plans accordingly.
Patient Education :
Provide clear instructions on wound care management to patients and their families.
Infection Prevention and Management :
Use proper infection control practices to reduce the risk of infection.
Collaboration :
Work closely with other healthcare professionals, including primary care providers, physicians, dietitians, and physical therapists, to ensure comprehensive care.
Documentation and Reporting :
Maintain accurate, up-to-date records of patient assessments, treatments, and progress.
Mobile Care Delivery :
Travel to patients’ homes or healthcare facilities to provide care as needed, following established safety protocols and professional standards.
Preventative Care :
Identify patients at risk for wound development and implement preventative measures.
Clinical Supervision :
Mentor and support other nursing staff involved in wound care.
Qualifications :
Education : Board certification as a Nurse Practitioner (NP), preferably with a specialization in wound care.
Licenses & Certifications :
Current, active NP license in [MI].
Experience :
Minimum of 2 years of clinical experience in wound care, home health, or similar healthcare settings.
Physical Requirements :
Ability to travel to patient homes and healthcare facilities.
