What You'll Do:
As the Care Coordinator, you’ll be the heartbeat of our scheduling and communication flow, ensuring smooth coordination between caregivers, clients, and our leadership team.
✨ Match caregivers to clients based on skill, availability, and compatibility.
✨ Maintain and update the master schedule with real-time accuracy.
✨ Serve as the primary point of contact for caregivers regarding scheduling, shift changes, and client updates.
✨ Provide excellent communication and service to clients and their families—empathy is everything.
✨ Collaborate with the HR and leadership team to ensure all client plans of care are staffed and supported.
✨ Document caregiver incidents, call-offs, and changes, and escalate when needed.
✨ Support orientation, training reminders, and compliance tracking as needed.
✨ On occasion, assist with intakes, care plan updates, or in-home visits (as assigned by leadership).
✅ What You Bring:
✔ Experience in home care or a healthcare scheduling role (required).
✔ High School Diploma or GED (Associate's or higher preferred).
✔ Excellent communication and customer service skills—you're calm, kind, and clear.
✔ Ability to multitask and work well under pressure in a fast-paced environment.
✔ Strong tech skills: comfortable using scheduling software, spreadsheets, and EMR systems.
✔ A heart for service and a mind for coordination.
