This is a full time position in the Social Services Division within Wright County Health & Human Services (WCHHS).  This position serves as a housing subject matter expert, develops professional working relationships with community housing providers, and provides assistance to individuals experiencing homelessness.  This position is responsible for community networking and coordination specific to housing resource and development needs related to WCHHS, while also providing short-term case management support to individuals experiencing homelessness to assist them in securing housing.  

Performs difficult advanced human support work providing social services planning, coordination and mental health case management and expertise, and related work as apparent or assigned.  Uses research, data analysis and evaluation to assist Social Service Managers, Supervisors, and Staff in guiding effective social services programs. Work is performed under the general direction of the Social Services Supervisor.   Oversight is exercised over personnel within the team.
 
This position may be assigned to specialized functions within the Health and Human Services Department.

  • Serves as a Team Leader to co-workers and providing training and work direction, program development, coordinates annual and ongoing divisional training.
  • Coordinates data management activities for multiple social service programs including demographic, programmatic, socio-economic, environmental and other pertinent data; Coordinates, monitors and evaluates program improvement initiatives and makes recommendations based on trends and analysis.
  • Consults with referring sources and other professionals; reviews, completes and submits reports, forms and paperwork.
  • Provides early intervention in collaboration with schools, law enforcement, hospitals and other community partners to provide resource, referral information and short-term case management.
  • Perform clinical evaluations and assessments to establish need for initial or continued service under clinical supervisor.
  • Maintains knowledge of trauma-informed and generalist practice to serve as a mental health resource provide staff consultation and perform case management and client related services. Meets with clients and Department staff to identify client needs and set therapeutic goals; develops treatment plans; monitors and adjusts as necessary, while under clinical supervision.
  • Reports incidences of maltreatment, death and serious injuries of adults and children; testifies and provides documentation in legal proceedings.
  • Maintains knowledge of programs, laws and procedures by attending meetings, training and conferences, and reviewing technical literature.
  • This position may be assigned to assist law enforcement personnel on coordinated emergency responses to assist with mental health and/or substance abuse calls within the County.
  • Performs other duties and activities as assigned.
Education and Experience
Bachelor’s degree in social work, counseling, psychology, or related field and three years of social worker or related experience or a master’s degree and one year of social worker or related experience.

Special Requirements
Any Social Worker II assigned to assist law enforcement personnel with coordinated emergency responses is required to pass a BCA background check.
A valid driver’s license or evidence of equivalent mobility is also required upon hire.

Knowledge, Skills, and Abilities
Knowledge of mental health therapies and psychological disorders; knowledge of human behavior principles and theory; knowledge of, and ability to interpret and apply, laws and regulations related to mental health and chemical dependency programs;  knowledge of court procedures and practices; knowledge of conflict management and ability to recognize and diffuse conflict; knowledge of, or ability to learn, computer systems and programs used to administer Human Service programs; knowledge of, or ability to learn, County government work flows and processes; skill in the use of standard office equipment, including computers and software programs; verbal and written communication skills sufficient to effectively present information and respond to questions from a wide variety of audiences; reading comprehension skills sufficient to read and understand medical reports and technical and legal documents; interpersonal skills sufficient to exchange and or convey information, receive work direction, and maintain effective working relationships; ability to perform data entry into various databases; ability to pay attention to detail and effectively organize time and anticipate, plan and successfully respond to changing circumstances; ability to perform all job duties in compliance with safety guidelines and with an ongoing awareness of safety practices; ability to communicate courteously and professionally while maintaining positive working relationships with others when carrying out all job functions.

This work requires the occasional exertion of up to 25 pounds of force; work regularly requires speaking or hearing and using hands to finger, handle or feel, frequently requires standing, reaching with hands and arms and repetitive motions and occasionally requires walking, sitting and lifting; work has standard vision requirements; vocal communication is required for expressing or exchanging ideas by means of the spoken word and conveying detailed or important instructions to others accurately, loudly or quickly; hearing is required to perceive information at normal spoken word levels and to receive detailed information through oral communications and/or to make fine distinctions in sound; work requires preparing and analyzing written or computer data, operating machines, operating motor vehicles or equipment and observing general surroundings and activities; work occasionally requires exposure to fumes or airborne particles, exposure to toxic or caustic chemicals; work occasionally requires exposure to outdoor weather conditions; work is generally in a moderately noisy location (e.g. business office, light traffic).
