Job Summary
We are seeking a dedicated and compassionate Case Manager to join our team. The ideal candidate will play a crucial role in supporting individuals and families by assessing their needs, developing personalized care plans, and coordinating services to ensure optimal outcomes. This position requires a strong understanding of behavioral health, child welfare, and crisis management, as well as the ability to work collaboratively with various stakeholders.
Responsibilities
- Conduct comprehensive assessments to identify client needs and develop individualized care plans.
- Coordinate services and resources for clients, including healthcare, behavioral health, and social services.
- Provide crisis intervention and management support to clients in need.
- Supervise and mentor junior staff or interns as needed.
- Maintain accurate documentation of client interactions and progress in compliance with organizational policies.
- Collaborate with community organizations, healthcare providers, and other stakeholders to ensure holistic support for clients.
- Monitor client progress and adjust care plans as necessary to achieve desired outcomes.
- Advocate for clients’ rights and access to necessary services.
Experience
- A background in social work, behavioral health, or related fields is preferred.
- Experience with Medicare processes and regulations is a plus.
- Familiarity with childcare and child welfare systems is beneficial.
- Previous experience in addiction counseling or crisis intervention is highly valued.
- Strong supervisory skills with experience mentoring others are advantageous.
- Excellent communication skills, both verbal and written, are essential for effective client interaction.

Job Type: Part-time
Pay: $20.00 - $25.00 per hour
Expected hours: 10 – 20 per week
