You’ll lead the development and implementation of a comprehensive care management program that includes case management, utilization review, discharge planning, and social work. Key Responsibilities
- Lead and supervise case management services, including acute care, rehabilitation, and outpatient care.
- Manage department operations, including productivity, staffing, and financial oversight.
- Coordinate utilization management (UM) committee activities and reports.
- Act as a clinical resource for hospital departments and community liaison with external providers.
- Hire, mentor, and manage the performance of case management staff, fostering professional growth.
- Oversee Key Performance Indicators (KPIs) for quality-focused case management.
- Education: Bachelor’s Degree required.
- Experience:
- 5 years in Nursing or Social Work, with at least 3 years in hospital-based Case Management.
- Supervisory/management experience in Case Management required.
- Acute care setting experience (discharge planning, clinical utilization review) preferred.
