Position Summary

We are seeking a Director of Care Management to oversee care coordination across two facilities. You’ll lead the development and implementation of a comprehensive care management program that includes case management, utilization review, discharge planning, and social work. Your leadership will play a key role in ensuring high-quality patient outcomes, efficient department operations, and compliance with key performance indicators.

Key Responsibilities

  • Lead and supervise case management services, including acute care, rehabilitation, and outpatient care.
  • Manage department operations, including productivity, staffing, and financial oversight.
  • Coordinate utilization management (UM) committee activities and reports.
  • Act as a clinical resource for hospital departments and community liaison with external providers.
  • Hire, mentor, and manage the performance of case management staff, fostering professional growth.
  • Oversee Key Performance Indicators (KPIs) for quality-focused case management.

Qualifications

  • Education: Bachelor’s Degree required.
  • Experience:
    • 5 years in Nursing or Social Work, with at least 3 years in hospital-based Case Management.
    • Supervisory/management experience in Case Management required.
    • Acute care setting experience (discharge planning, clinical utilization review) preferred.
