What You’ll Do :
Deliver compassionate, high-quality care in patients' homes.
Complete OASIS assessments and maintain detailed documentation.
Work alongside a dedicated care team to develop personalized treatment plans.
Educate patients & caregivers on managing conditions and medications.
Track patient progress and communicate any health changes promptly.
What We’re Looking For :
Experienced Home Health RN with a passion for patient care.
Proficiency with EHR systems used in home health.
Strong time management & organizational skills.
