General Description
The individual selected will perform duties at an advanced level, under general supervision, by providing services to assigned patients within the psychiatric hospital setting. The individual selected will be responsible for social work and treatment planning duties for assigned case load and will be assisting other hospital social workers and treatment planners as needed. Individual will work with treatment teams to develop comprehensive psychosocial assessments that evaluate and consider components of a patient's life to establish a thorough history of information needed for treatment of his/her mental illness. Will assist in the development, organization, and progression of individualized plan of care from admission to discharge. Will assist in the sharing of appropriate information to family, legal representative, advocacy, etc. Will help aid in the continuity of care and transition from the hospital setting to the community. Will complete up to date documentation and communicate findings, resource information, and all other pertinent information with treatment team members. Individual will link and refer patients to services to support their needs and network with the community to help identify agencies that can later become resources to patients in need; facilitate the successful discharge of a patient to a setting that supports their individualized needs. Will work closely with the professional treatment team including psychiatrist, psychologist, registered nurse, families and behavioral health centers staff daily. Performs related duties as required. Mandatory overtime may be required depending on the operational needs of the hospital and patients.

Minimum Qualifications
Education: Bachelor's degree in Social Work, Sociology, Psychology, Counseling, Criminal Justice, Behavioral Science, Human Services, Education, or related field from an accredited college or university.
Licensure/Certifications: Eligible for Temporary Social Work License OR Licensed as a Social Worker, Graduate Social Worker, Provisional Social Worker, or Certified Social Worker by the West Virginia Board of Social Work Examiners.
Experience: One (1) year of full-time or equivalent part-time experience performing social work in a healthcare setting, working directly with individuals.
