MINIMUM QUALIFICATIONS:
EDUCATION, CERTIFICATION, AND/OR LICENSURE:
- Current unencumbered licensure with the WV Board of Registered Professional Nurses, or appropriate state board where services will be provided, as a registered professional nurse OR current multi-state licensure through the enhanced Nurse Licensure Compact (eNLC).
- Obtain Certification in area of specialty within 1,000 hours of documented diabetes education teaching.
- Obtain certification in Basic Life Support within 30 days of hire date.
- Two (2) years of clinical experience with One (1) year in the care of diabetic patients.
EDUCATION, CERTIFICATION, AND/OR LICENSURE:
- Bachelor’s degree in Nursing (BSN).
- Certification in Diabetic Education upon hire.
- Five (5) years of clinical experience in nursing.
- Identifies education needs of the community and those with diabetes.
- Attends diabetes advisory committee meetings as assigned.
- Performs patient assessment, delivers education and evaluates patient’s obtainment of goals.
- Assists in maintaining the American Diabetes Association certification as applicable.
- Participates in community outreach and educational support.
- Advocates on behalf of the patient and families.
- Communicates with primary care providers, members of the multi-disciplinary team relevant questions concerns, and patient outcomes.
- Participates in performance improvement activities to assure quality patient care.
- Completes daily rounds to assess patient status.
- Ensures all necessary testing and scheduling is complete prior to necessary procedures.
- Compiles reference materials, summarizes data and prepares reports, and drafts manuals and materials.
- The National Institute for Occupational Safety and Health recommends minimizing the need for employees to manually lift patients and limit safe patient handling to 35 pounds. All transfers, lifts and re-positioning will be completed utilizing the lift equipment and/or other patient handling aids as indicated by the patient’s profile and appropriate algorithms.
- Heavy/Hard work: Work requires strength and/or stamina, lifting, moving, stooping, reaching, standing, walking, and carrying of materials and equipment weighing 35 lbs

- Ability to communicate clearly with patients, families, physicians, and other employees.
