MINIMUM QUALIFICATIONS:
EDUCATION, CERTIFICATION, AND/OR LICENSURE:
- Bachelor’s degree in Social Work, Psychology, Sociology, Nursing, or related field.
- West Virginia state criminal background check, and federal, if applicable, required for DHHR regulated area.
EXPERIENCE:
- One (1) year experience in a healthcare setting.
- Participates as an integral professional member of an interdisciplinary team.
- Collects and records data that is comprehensive, accurate and systematic, on the psychosocial assessment form and in progress notes. Correlates all assembled data on the psychosocial assessment and history in a manner that is clinically pertinent to the individual patient.
- Assesses and documents in the medical record the biopsychosocial needs of the patient, according to the development age of the patient.
- Contacts the referral source(s) and appropriate health professional involved in patient’s care to inform them of patient status and to obtain any additional information that might facilitate the treatment process.
- Advocates on behalf of the patient to maximize available services.
- Participates in treatment planning sessions by assisting in development of master treatment plan with specific goals and interventions delineating nursing and social work actions unique to each client’s needs and developmental age. Assists treatment team by coordinating planning effort. Assists in determining appropriateness of admissions and receives referrals for social work intervention.
- Establishes and maintains a therapeutic milieu in collaboration with the patient and other health care providers, taking developmental age of patient into account.
- Assists patients and families in achieving a satisfying and productive pattern of living through health teaching and documents teaching according to hospital policy.
- Performs discharge planning as developed with the treatment team from day of admission. Acts as liaison for patients and their families to various community agencies, hospital consultants and services.
- Supervises/transports patients to appointments and activities on and off the unit.
- Participates in improving organizational performance and other means of evaluation to assure quality of nursing and social work services provided for patients.
- Attends continuing education seminars and inservices as necessary to maintain professional licensure, competency and to satisfy hospital required educational standards.
- Frequent walking, standing, stooping, kneeling, reaching, pushing, pulling, lifting, grasping are necessary body movements utilized in performing duties through the work shift.
- When working on inpatient units must be able and willing to participate in physical restraint of patient if needed.
- Healthcare setting.
- Must have independent decision-making ability.
- Ability to work with multi-disciplinary groups and facilitate meetings.
- Ability to work independently or cooperatively as a team member.
