Additional Job Description:

POSITION SUMMARY: The Advance Practice Professional (APP) Fellow is an individual charged with their development of clinical urology expertise. The APP Fellow completes comprehensive patient health evaluations (including biological, social, and psychological assessments) as assigned by appropriate clinical/faculty team. The APP Fellow assesses and coordinates the educational and psychosocial needs of the urology patient and caretakers. The APP Fellow develops and reviews the plan of care with the other members of the clinical team, participates in directing that care, and coordinates peri-operative care, discharge planning (as applicable) and appropriate post hospital and ambulatory follow up. The APP Fellow will participate in educational curriculum as outlined in the program.

MINIMUM QUALIFICATIONS FOR NURSE PRACTITIONER:

Provide healthcare services consistent with state law and the requirements of the professional licensing or certification authority and/or hospital privileges, where applicable. Such service may include but are not limited to:

EDUCATION, CERTIFICATION, AND/OR LICENSURE:

  • Graduate of a CCNE accredited nursing program with Master’s Degree in Nursing.
  • National Certification by one of the recognized APRN Certification Centers.
  • Licensure; or eligibility for licensure, as a professional Registered Nurse and Advanced Practice Registered Nurse in West Virginia.
  • American Heart Association BLS Certification required with additional Advanced Life Support training at the discretion of the

department.

EXPERIENCE:

  • Urology experience preferred.
  • Eligible new graduates will be considered.

PREFERRED QUALIFICATIONS:

EDUCATION, CERTIFICATION, AND/OR LICENSURE:

  • Bachelor ’s Degree as Physician Assistant or Master ’s Degree as Physician Assistant required.
  • Graduate of an ARC-PA approved Physician Assistant (PA) training program required.
  • Certification by the National Commission for Certification of Physician Assistants required.
  • Must be licensed or eligible for licensure by the WV Board of Medicine or WV Board of Osteopathic Medicine.
  • American Heart Association BLS Certification required with additional Advanced Life Support training at the discretion of the

department.

EDUCATION, CERTIFICATION, AND/OR LICENSURE:

  • Master’s level degree as Physician Assistant preferred.

CORE DUTIES AND RESPONSIBILITIES: The statements described here are intended to describe the general nature of work being performed by people assigned to this position.   • Manages comprehensive and individualized care for urology patients and their families in the inpatient phase of hospitalization (as applicable), immediate post discharge, and in the ambulatory setting in conjunction with Fellowship leadership and/or the urology care team.
  • Completes daily rounds; consults and/or pre-procedure urology evaluations as assigned.
  • Completes integrated assessments as assigned.
  • Participates in peri-operative management of patient as assigned.
  • Orders additional diagnostics or consults as deemed appropriate with the collaboration of urology providers or by following established Fellowship guidelines.
  • Performs and communicates results of diagnostics following established Fellowship guidelines and scope of practice.
  • Consults and communicates with clinic and hospital staff regarding abnormal findings in the evaluations of patients.
  • Performs succinct comprehensive clinical evaluations and documents findings in medical record as assigned.
  • Facilitates patient discharge/follow up by coordinating appropriate services and diagnostics prior to discharge necessary for outpatient follow-up appointments in the clinic (as applicable) as assigned.
  • Performs necessary procedures following established Fellowship guidelines for urological scope of practice (SOP).
  • Develops a comprehensive individualized plan of care for urology patients, which includes teaching, discharge planning/follow-up and necessary referral to other members of the health care team as assigned.
  • Demonstrates expertise in caring for patients across the age continuum of their educational experience and certification.
  • Participates in patient rounds and consultations (as assigned).
  • Identifies unstable or complex urology patient care situations and communicates to Fellowship leadership and/or clinical providers to initiate appropriate intervention in the clinic and/or hospital setting.

Facilitates patient discharge and/or postoperative care by coordinating appropriate services and diagnostics prior to scheduled outpatient follow up appointments (as assigned).

  • Assists in the collection of outcome measures (as assigned).
  • Recognizes emergency situations and initiates effective emergency care in collaboration with clinical team.
  • Evaluates the effectiveness of the urology patient/family teaching and collaborates with urology care team in the completion of education plan.
  • Demonstrates the development of critical thinking and diagnostic reasoning skills for clinical decision making by the identification, evaluation, and management of the individual needs of patients/families.
  • Demonstrates the developed knowledge to utilize equipment in a safe manner.
  • Develops decisions based on relative scientific principles, established standards of care and/or research findings.
  • Utilizes learning theory and appropriate educational strategies to facilitate collaboration with nursing staff, hospital staff, patients, families, and the community.
  • Promotes an environment that facilitates learning by responding to appropriate patient/family preferences, priorities, and readiness to learn.
  • Updates own knowledge base throughout the year.
  • Completes all identified clinical competencies.
  • Serves as a resource for collaboration with the urology care team.
  • Provides educational presentations to professional and paraprofessional personnel as assigned.
  • Participates in community programs and/or distribution of literature as assigned.
  • Communicates effectively with patients/families, providers, and ancillary services at all levels to assure coordinated efficient patient care.
  • Documents the plan of care, interventions, and evaluations in a clear and concise manner for every patient visit in the patient’s medical record following approved hospital/UHA policies and compliance standards for documentation.
  • Collaborates with other Fellowship leaders and care team to effectively achieve patient outcome-based improvements in care.
  • Participates in the development and implementation of changes in the urology practice that positively impact patient outcomes.
  • Works collaboratively with urology care team to facilitate creative problem solving.
  • Participates in interdisciplinary approaches to patient care, program development and education, as assigned.
  • Promotes and practices innovation of the expanded role in the delivery of care to urology patients and families throughout the inpatient hospitalization, and the ambulatory setting.
  • Participates in committees and task forces as assigned.
  • Documents hours and submits billing reports, as appropriate.
  • Participates in the WVUM/UHA Performance Improvement Plan.
  • Participates in clinical trials and research studies as assigned.
  • Other duties/projects as assigned by Fellowship leadership.
