Job Description:
The Advanced Practice Professional (APP) plays a crucial role in the comprehensive management of family medicine patients. This includes biological, social, and psychological assessments, coordination of medical specialty teams, and review of the plan of care with the clinical team.
- Manage individualized care for family medicine patients in the ambulatory setting.
- Consult and/or pre-procedure/evaluations, acute visits, and wellness visits.
- Complete integrated assessments and order additional tests or consults as deemed appropriate.
- Perform and communicate results of diagnostic tests following established clinical guidelines and scope of practice.
Requirements:
To be considered for this position, you must have:
- A Master's Degree in Nursing from a CCNE accredited nursing program.
- National Certification by one of the recognized APRN Certification Centers.
- Licensure or eligibility for licensure as a professional Registered Nurse and Advanced Practice Registered Nurse in West Virginia.
- BLS Certification required with additional Advanced Life Support training at the discretion of the department.
Preferred Qualifications:
Prior experience in family medicine is preferred. Eligible new graduates will also be considered.
Responsibilities:
As an APP, your responsibilities will include:
- Developing a comprehensive individualized plan of care for family medicine patients.
- Caregiving for patients across the age continuum.
- Facilitating patient follow-up by coordinating appropriate services and tests for follow-up appointments.
- Documenting the plan of care, interventions, and evaluations in a clear and concise manner.
Work Environment:
This role requires working closely with others and protracted or irregular hours may be necessary. A physical presence on site is required.
Schedule: 40 hours per week. Exempt/Non-Exempt: United States of America (Exempt)
