**Job Title:** Advanced Practice Professional (APP)
The Advanced Practice Professional (APP) has coordination and clinical skills required for the comprehensive management of primary care patients, including biological, social, and psychological assessments.
The APP coordinates involvement of medical specialty teams necessary to develop a comprehensive plan for the patient, and reviews the plan of care with other members of the clinical team. They participate in directing that care, perform diagnostic/therapeutic procedures, and coordinate discharge planning as applicable.
The APP participates in the education of residents and medical/nursing students.
**Requirements:
- Graduate of a CCNE accredited nursing program with Master's Degree in Nursing
- BLS Certification required with additional Advanced Life Support training at the discretion of the department
- Primary care experience preferred
- Eligible new graduates will be considered
Responsibilities:
- Manage comprehensive and individualized care for primary care patients and their families in the ambulatory setting
- Order additional tests or consults as deemed appropriate, collaborating with primary care faculty or following established clinical guidelines and protocols
- Consult and communicate with clinic and hospital staff regarding abnormal findings in patient evaluations
- Perform succinct comprehensive clinical evaluations and document findings in the medical record
- Develop a comprehensive individualized plan of care for primary care patients, which includes teaching, follow-up planning, and necessary referral to other members of the healthcare team as necessary
- Identify unstable or complex primary care patient care situations and facilitate appropriate intervention in the clinic and/or hospital setting
- Evaluate primary care patients and serve as a coordinator for follow-up procedures and tests in the ambulatory setting
- Assist in the collection of outcome measures
- Demonstrate the ability to utilize equipment in a safe manner
- Base decisions made/actions taken on relevant scientific principles, established standards of care, and/or research findings
- Utilize teaching/learning theory and appropriate teaching strategies in providing education to nursing staff, hospital staff, students, patients, families, and the community
- Serve as a resource person for staff caring for primary care patients
- Participate in community programs, distribution of literature, as appropriate in area of expertise
- Document the plan of care, interventions, and evaluations in a clear and concise manner for every patient visit in the patient's medical record following approved hospital/SMG policies and compliance standards for documentation
- Collaborate with other leaders and staff to effectively achieve patient outcome-based improvements in care
- Provide leadership in the development and implementation of changes in the primary care practice that positively impact patient outcomes
- Work collaboratively with medical and nursing staff to facilitate creative problem-solving and provide professional support
- Promote and practice innovation in the expanded role in the delivery of care to primary care patients and families throughout the inpatient hospitalization and the ambulatory setting
- Document hours and submit billing reports as appropriate
- Review and interpret primary care data from sources such as patient registries, clinical and financial reports, length of stay, access to care, and cost per case reports and recommend/implement changes as necessary
- Assist primary care Faculty, Administrators, and Directors in the collection of outcome measures
