Job Summary
The registered nurse case manager is responsible for the direct and indirect nursing care of patients in their homes. In collaboration with the patient, family, and healthcare team, the nurse develops, implements, and evaluates nursing care according to agency and system policies.
Responsibilities
- Provide one year of experience in home care nursing and two years of prior nursing experience.
- Perform the functions of a home health aide.
- Maintain basic patient care according to policies and procedures.
- Assume on-call responsibility per policy and procedure.
- Communicate patient care problems or potential problems to the administrative staff and other interdisciplinary team members as identified/occur to ensure continuity and coordination of care.
- Provide care based on physical, psychosocial, communication, safety, education level, and related criteria for infant, pediatric, adolescent, adult, geriatric patients per established age-specific education/standard.
Requirements
- Complete and document full system assessment, including assessment of level of physical and psychosocial aspects and identifies age-specific criteria upon admission and each visit thereafter.
- Understand pathophysiologic process by taking appropriate nursing action in accordance with changes in patient condition.
- Report significant observed changes in patient condition to administrative staff and/or physician.
- Document patient status and care or services furnished in appropriate areas defined by documentation policy and procedure.
- Document home health aide supervisory visits as per standard.
Skills
- Demonstrate bag technique per policy and procedure.
- Able to apply and maintain equipment according to the Policy and Procedure Manual.
- Vital signs, patient weights, specimen collection, ostomy care, catheterization, wound care, dressing changes, suctioning, gastric tube feedings and maintenance.
- Perform and monitor patient teaching according to policy and procedures.
- Accurately and timely complete plan of treatment and recertification as per Medicare guidelines.
