Job Summary:

The MDS Coordinator is responsible for overseeing the completion and submission of the Minimum Data Set (MDS) assessments in compliance with federal and state regulations. This role ensures accurate and timely documentation to support patient care planning, reimbursement, and quality reporting. The MDS Coordinator collaborates with interdisciplinary teams to ensure that the care provided aligns with the assessed needs of residents in a skilled nursing facility.

Pay: $83k-$93k

Key Responsibilities:

MDS Assessment Coordination:

  • Coordinate and complete MDS assessments (including initial, quarterly, annual, and significant change assessments) within required timeframes.
  • Ensure accuracy and completeness of MDS data, including coding and care area assessments (CAAs).
  • Submit MDS assessments to the appropriate state and federal databases.

 Compliance and Regulatory Knowledge:

  • Stay updated on federal and state regulations related to MDS, Resident Assessment Instrument (RAI), and Medicare/Medicaid reimbursement.
  • Ensure compliance with CMS guidelines, state-specific requirements, and facility policies.

Interdisciplinary Collaboration:

  • Work closely with nursing, therapy, social services, dietary, and other departments to gather necessary information for MDS completion.
  • Participate in care planning meetings to ensure MDS data aligns with resident care plans.

Reimbursement and Quality Reporting:

  • Utilize MDS data to support accurate reimbursement through Medicare, Medicaid, and other payers.
  • Monitor and analyze quality measures (QMs) and quality indicators (QIs) to identify areas for improvement.
  • Assist in preparing for audits and surveys related to MDS and reimbursement.

Training and Education:

  • Provide training and support to staff on MDS processes, documentation requirements, and best practices.
  • Educate team members on the impact of MDS data on resident care and facility outcomes.

Data Management and Analysis:

  • Maintain organized and up-to-date MDS records and documentation.
  • Analyze MDS data to identify trends, improve care delivery, and support decision-making.

Resident-Centered Care:

  • Ensure MDS assessments reflect the individualized needs and preferences of residents.
  • Advocate for residents by ensuring accurate representation of their clinical condition and care needs.

Qualifications:

Education:

  • Licensed Vocational Nurse (LVN) with a valid state license.
  • Certification in MDS (e.g., RAC-CT) preferred.

Experience:

  • Minimum of 2-3 years of experience in long-term care or skilled nursing facilities.
  • Proven experience with MDS 3.0 and RAI processes.

Skills:

  • Strong knowledge of Medicare/Medicaid reimbursement and PDPM (Patient-Driven Payment Model).
  • Excellent attention to detail and organizational skills.
  • Proficient in electronic health records (EHR) and MDS software.
  • Strong communication and interpersonal skills for effective collaboration.

Salary : $83,000 - $93,000
