What are the responsibilities and job description for the Supervising Behavioral Health Clinician position at Wyckoff Heights Medical Center?
Conduct initial pre-screening of clients to determine eligibility for services and appropriateness of case management services.
* Completes intake, assessment, development of service plans, and referrals.
* Triage of emergent situations, phone conversations, on-call responsibilities, and the ability to negotiate healthcare and social service systems.
* Provide basic HIV education, including disease progression, treatment, and prevention to patients and their support system.
* Maintains accurate and timely program case records, documentation and support reporting to DOHMH.
* Advocates for clients to obtain the full range of needed services and ensures coordination of these services.
* Promotes linkage development and monitors the effectiveness of linkage with other service providers.
* Ensures community follow-up to engage the client in care, promote compliance with medical appointments, and encourage client self-sufficiency and empowerment.
* Participates in multidisciplinary case conferences with all that are involved with clients, obtaining case records when necessary.
* Meets monthly with clients and more often as needed for counseling, follow-up, and advocacy.
* Refer and link clients to appropriate services within the system of care that promotes greater self-sufficiency. Monitor the client's follow-through with these services.
* Evaluate the effectiveness of services based upon clients outcome in the scope of work.
* Serves as a team member on a multidisciplinary medical care team, communicating with team members to ensure comprehensive and quality wrap-around support.
* To provide and coordinate behavioral health care management for patients in collaborative care (integrated primary care / behavioral health).
* Competency in DSM 5, ICD-10, and use of CPT codes.
* Conduct mental health assessments of patients in the Ambulatory clinics.
* Provides psychoeducation of patient on treatment options for behavioral health.
* Consults with patient and medical provider about treatment options and preferences, coordinates initiation of treatment plan.
* Provide brief, structured psychotherapy and behavioral activation to address depression and anxiety.
* Maintains an electronic registry of all patient participating in the program.
* Monitors patient closely (in person or by phone), using standardized measures, for changes.
* Coordinates communicates between patient, medical provider, and consulting psychiatrist/psych NP.
* Document all encounters in concise and accurate manner according to organizational policies and procedures.
* Participates in QI activities as assigned, and performs within acceptable limits on QA/QA indicators. Recommends training and maintains a tracking system of all trainings completed by assigned LMSW's and / or LMHC's and ensure they remain up to date in current evidenced based interventions.
* Arrives punctually for work, uses leave responsibly and judiciously, ensure coverage when absent.
* Complies with required standard for medical/legal reporting situations (e.g. child abuse, sexual assault).
* Ability to work with high degree of independence in a fast-paced environment.
* Serves as a Senior Clinician and Clinical Supervisor for assigned LMSW's and/or LMHC's.
* Participates in regular caseload supervision with assigned LMSW's and/or LMHC's to provide recommendations and guidance on patient treatment plans.
* Regularly reviews the progress notes and electronic registry entries for any assigned LMSW's and/or LMHC's to provide coaching and feedback to ensure compliance with documentation standards.
* Creates and reviews data collection mechanism to ensure program deliverables are routinely tracked and appropriate data is collected.
* Ensure any assigned LMSW's and /or LMHC's are meeting enrollment and service plan goals, and provides support and problem solving when challenges arise.
* Functions as a Clinical Lead for patients experiencing crisis in the Ambulatory clinics.
* Provides formal or informal teaching / consultation to health professionals and community, as requested.
* Perform other duties as assigned.
* Knowledge of supportive housing, HIV/AIDS, and Substance abuse is required.
* Experience in assessment, service planning, and community referrals.
Position Requirements:
* This position requires a minimum formal education of Bachelor's Degree and a minimum of 3 years job-related experience.
* Desired (not required) criteria include: Bilingual (English/Spanish). Minimum 3.5 years' experience providing mental health counseling & supportive services to MSM clients & clients living with HIV or at risk of HIV. Prior experience in brief, structured, evidence-based psychotherapy.
* New York State current LCSW License.
* Ability to work with a high degree of independence in a fast-paced environment & as a member of an interdisciplinary team. Strong written and verbal communication skills. Good problem solving & conflict resolution skills.
