Wyoming Valley Behavioral Hospital is setting the stage for a new beginning. Formally known as First Hospital, WVBH will be in the same building but have a fresh perspective and approach. We are a 149-bed free standing psychiatric inpatient facility located in Kingston Pennsylvania. Family friendly and staff focused will be our new mantra. Healing our patients with a combination of compassion medication and therapy, allowing for a successful recovery and reentry into their regular routine. We are looking for a case manager who will work directly with our director of social services on patient care planning, discharge planning, advocating for patient rights and more! Candidates should display calm, confidence interventions and enhances an environment of compassion to people seeking help for serious symptoms of mental illness . A great candidate should be aware of their approach to developing a healing environment in a hospital setting. This person will exhibit high standards of care, passion, and knowledge of mental illness as they render care and assistance to patients in a psychiatric facility and help maintaining accurate patient records according to policy. The candidate must be familiar with Trauma Informed Care theory and practice.

Responsibilities :
- Coordinates patient care, assesses, evaluates and updates care plans
- Connects patients with appropriate aftercare services and resources
- Assist patients and their families navigate referral system obstacles
- Maintain accurate and up-to-date medical records
- Advocate for patients' rights and safeguards as well as ensure access to equitable needed services and support resources
- Other duties as assigned by Director of Social Services
