Responsibilities
- Assist clients with daily living activities, including personal hygiene, meal preparation, and medication management.
- Develop and implement individualized care plans tailored to meet the unique needs of each client.
- Support clients in social interactions and community engagement to promote inclusion and enhance social skills.
- Maintain accurate documentation of client progress and any changes in behavior or health status while adhering to HIPAA regulations.
- Interact with individuals and family members in a professional and respectful manner, representing our organization positively.
- Collaborate with healthcare professionals, family members, and other support staff to provide comprehensive care.
- Provide emotional support and companionship to clients, fostering a nurturing environment.
- Assist with transportation needs for clients attending appointments or community activities.
- Maintain necessary certifications, including CPR, First Aid, Medication Assistance, and TOVA.
- Ability to commute and have transportation.
Skills
- 1 year experience in home care or working with individuals with disabilities.
- Knowledge of care plans, patient care practices, and resident rights is essential.
- Familiarity with HIPAA regulations to ensure confidentiality and privacy of client information.
- Strong interpersonal skills to effectively communicate with clients, families, and team members.
