NHTD Case Coordinator - Per Diem
Pay : $250 - $500 per case
X-Treme Care is a growing homecare company providing services throughout the Westchester area. We're looking for a NHTD Case Coordinator (per diem) to join our team and make a difference in the lives of those we serve.
Responsibilities :
- Provide NHTD / TBI applicants and participants with service coordination and case management
- develop and implement Initial Service Plans
- coordinate between service providers and participant
- monthly / Quarterly in-home visits
- orchestrate and document team meetings
- provide ongoing support
- Renewal Service Plans
