Group Home Social Workers
assist in the placement of people with Intellectual / Developmental Disabilities (I / DD) into
care homes, licensed as a California Group Homes, and
assisting with
various aspects of the support needs of the people
in the home.
The crisis home supports individuals on a short-term basis, twelve to eighteen (12-18) months, and seeks
to stabilize the them so that they can move into a longer-term living environment as quickly as possible and be successful
in that new living arrangement. The enhanced behavior home supports individuals to be successful in their living
arrangement.
Key / Essential Functions & Responsibilities :
- Assists in the completion of intake study, needs and services plan and discharge plan for each person in the home, obtaining, developing and recording the information necessary for the completion and ensuring compliance with all applicable regulations.
- Participates in the transitional planning process, including Individual Education Plan (IEP), Individual Program Plan (IPP), Individual Behavior Support Plan (IBSP) meetings and seeks out community resources that will best support individual needs.
o Ensures that each individual's written Needs and Services Plan is regularly reviewed and updated as often as necessary, but at least every six (6) months, to ensure its accuracy, and to document significant occurrences
that result in changes in the client's physical, mental, psychological, and / or social functioning.
of Developmental Services, Department of Social Services - Community Care Licensing Division and Regional Center and other entities such as hospitals, court systems, day programs and / or school districts.
Residential Council Meetings with the people who reside in the home, updating the Administrator, Qualified Behavior Modification Professional (QBMP) and Lead Registered Behavior Technicians (RBT) on the proposed monthly menu and activity schedule.

Minimum Qualification Requirements including education, experience, licensure / certification, etc. and essential physical
capabilities (e.g., lifting, assisting lifting, standing, etc.)
Travel to various regions and programs required.
