What are the responsibilities and job description for the Support Services Coordinator position at YAI?
At YAI, we are committed to delivering exceptional care and support services to individuals with developmental disabilities. As a Social Worker, you will play a vital role in ensuring that our clients receive the highest level of care and support.
The ideal candidate will possess a strong background in social work, with a focus on providing individualized support and services to individuals with developmental disabilities. In this role, you will be responsible for:
- Assessing and Planning: Conduct comprehensive assessments and develop personalized plans to meet the unique needs of each individual.
- Intake Assessments: Complete thorough intake assessments during the referral process, reviewing current and historical records, and performing direct observations and interviews with the individual and their support group.
- Collaboration: Participate in the transitional planning process, working closely with the Qualified Behavior Modification Professional (QBMP), and collaborate with interdisciplinary teams to ensure continuity of care.
- Documentation: Maintain accurate and up-to-date records of client services, including Needs and Services Plans, and ensure compliance with relevant regulations and timelines.
You will also have the opportunity to:
- Liaison Role: Act as a liaison between the home and various parties on behalf of the individuals we support, including conservators, advocates, and oversight agencies.
- Training and Education: Provide training and education to group home staff regarding support needs and services, and conduct additional trainings as requested by Program Administrators or clinical supervisors.
- Residential Council Meetings: Regularly conduct Residential Council Meetings with individuals, updating staff on proposed monthly menus and activity schedules.
We offer a competitive salary of $37.00-$37.00 and require the following qualifications:
- Bachelor's Degree in Social Work from an accredited program, or equivalent experience in human services.
- CPR/First Aid, Ukeru, and CPI certifications, with annual recertification.
- Knowledge of California's Lanterman Act and related service systems, Title 17 and/or Title 22 regulations.
- Exceptional verbal and written communication, interpersonal, and presentation/training skills.
