- Assess patients' physical and psychosocial condition on an ongoing basis throughout the continuum of care, identifying areas for improvement and developing strategies to address these needs.
- Implement evidence-based practices and guidelines to ensure compliance with regulatory requirements and hospital policies.
- Collaborate with healthcare providers to develop and implement individualized plans of care that address patients' unique needs and goals.
- Maintain accurate and up-to-date records of patients' health information, including medical history, surgical history, medications, allergies, and preferred laboratory and pharmacy providers.
